The initial visit is scheduled for two
separate days. The second visit will be scheduled at
least twenty-four hours after the first visit to allow
time for the lab tests to come back.
We believe the best care can be given when the physician and the
patient interact on a one-to-one basis. Having other people ...
children, spouses, parents, friends, etc…. in the room is a
distraction to the patient and the provider alike. Therefore, we
prefer that you not bring other people back to the lab or exam room
with you.
THE FIRST DAY
is set
up for handling all of the tests. We suggest that these
tests be done while the patient is fasting. Therefore,
please do not eat or drink anything but water for
six to eight hours before the appointment. Please also
wear a two-piece outfit on both days, and do not apply any
body lotions, oils, or sprays. The following tests
will be performed:
Comprehensive
Metabolic Panel
CBC - complete
blood count
Thyroid
Profile and Free T3
Insulin
Urinalysis
EKG
Body
Composition - measures your muscle lean to fat ratio
This appointment takes approximately 45
minutes to one hour. Please fill out the enclosed
Patient Information and Questionnaire. Failure to
bring your completed paperwork may delay your
appointment. You will also be given a CD to listen to
before your second visit.
THE SECOND DAY
you will be seen by a licensed medical
provider (M.D., D.O., P.A. or A.R.N.P.) for a brief
history and physical (listening to your heart and lungs)
and discussion of any abnormal results from the lab tests.
These visits are scheduled Monday, Tuesday, and Thursday at 9:00
a.m. and 1:30 p.m., and Wednesday at 9:00 a.m. This appointment
will take approximately 45 minutes to one hour. Please arrive
prior to your appointment time.
The new patient visit is $350.00 for both days and we ask
that this be paid with Visa, MasterCard, Discover, cash, check or
money order.
If you are late your
appointment will need to be rescheduled. 24 hours notice needs to be
given for all cancellations or to reschedule your appointment; or a
service charge may be taken.
